In the claims application, we require the customer to upload the clinic receipt and proof of full payment. The clinic receipt must include the following information:


•    Microchip number (if any)

•    Your pet's name

•    Charge breakdown

•    The diagnosis

•    First symptom date

•    Vet signature and stamp

•    Your pet's age (if any)


We also need a histopathology report from your vet if you're filing a claim for the Cancer Cash Benefit.


Please keep the original receipts until the claim is paid, as we may need them for verification purposes. We may also require you to provide your pet's medical records, including examination and diagnostic test reports, etc, from all previous visits to veterinarians.
